Call 877-876-3266 to make an appointment so you know what date your passport application can be filed. Travisa will manage this appointment for you, and you will not need to be there in person on this date.

You can renew your US passport if:
- You are able to submit your most recent US passport and it is in good condition.
- Your most recent passport was issued in your current name.
- Your most recent passport was issued within the past 15 years.
- Your most recent passport was issued when you were 16 years of age or older.

Note: If your passport is damaged or mutilated, you must follow New Passport application procedures.

Proof of Citizenship:

Most recent U.S. passport. (Issued less than 15 years ago).

Photo Requirements:

Two passport photos (2x2 with white background). Click here for passport photo requirements.

Passport Application Form:

One completed application form DS-82.This form must be completed online, printed on two pages (not back-to-back) and signed by the applicant. The passport office will not accept hand-written DS-82 applications. This form must have the original signature of the applicant.

Authorization Letter:

Two signed letters of authorization. Call 877-876-3266 to make an appointment and receive your application kit and letters of authorization to sign.

Proof Of Departure:

Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft or urgent need of passport processing for business travel.

Fee Description:

US Government Fee: $170.00 payment included on the Global Service Order Form or payable in the form of a check made out to the U.S. Department of State.
