The U.S. State Department has formally announced the elimination of visa page inserts for US Passports effective January 1, 2016. Travelers in need of additional pages in their valid passports now must renew their passport. Note: If your passport is damaged or mutilated, or you do not meet these criteria, you must follow New Passport application procedures. If you need more visa pages, call 877-876-3266 to make an appointment for passport renewal.
Two signed letters of authorization. Call 877-876-3266 to make an appointment and receive your application kit and letters of authorization to sign.
